A member should contact their Kaiser Permanente plan’s member services team for assistance with their Kaiser Permanente membership.

A member may provide his/her change of information to us by contacting their Kaiser Permanente plan’s member services team. A member should also provide the change of information to his/her employing agency.

A member should contact their Kaiser Permanente plan’s member services team for assistance with obtaining a new or replacement ID card and to schedule care prior to receiving it.

If a member is currently enrolled and will continue to be enrolled under a Kaiser Permanente FEHB Self and Family plan, he/she may use our Kaiser Permanente Enrollment Change Form to either add or delete a dependent. The member should refer to the Brochures and Forms page > to download an enrollment form.

Paperless Reimbursement
- Kaiser Permanente participates in the FSAFEDS automatic reimbursement process, called Paperless Reimbursement, which automatically reimburses you for copayments and coinsurance paid to Kaiser Permanente for most covered medical and pharmacy services received (does not include dental services, eyeglasses and contact lenses and non-covered services). You will not need to submit receipts to FSAFEDS after receiving services.
- To enroll in Paperless Reimbursement, you must contact FSAFEDS at 877-FSAFEDS (or 877-372-3337) (TTY: 866-353-8058). Or, you may go to the FSAFEDS Program website, log in to your online account, select Reimbursement and Payment Options, and follow the instructions for Paperless Reimbursement.
Manual Claim Reimbursement
- You may submit your eligible out-of-pocket health care flexible spending account expenses online at FSAFEDS.com, on the FSAFEDS app, fax or mail. See the FSAFEDS Program website for information.
- Kaiser Permanente FEHB members must include both a Check-In Receipt and an After-Visit Summary per each medical service or office visit that is submitted for reimbursement to FSAFEDS:
- A Check-In Receipt is the copayment receipt provided after checking-in for your visit.
- An After-Visit Summary is a summary of your visit details given to you at the end of your office visit appointment and is accessible through your secure kp.org account.

Yes. You can choose and change your doctor anytime, for any reason.
We’ve carefully selected our doctors so you can make the right choice for you and your family. We have highly respected doctors from some of the top medical schools in the country.

You can use kp.org to browse our doctor profiles to see education, credentials, specialties, profile photos, and everything else you need to know to select a doctor who’s right for you. You can search by location, gender, specialty, and language spoken. And in most regions, you can call Member Services for help selecting a doctor. (In Hawaii, you can use kp.org to search for a doctor, but you’ll need to call the facility where you plan to receive care for help in making your selection.) You have many locations to choose from, and most of them offer multiple services under one roof. You can see your doctor, get a lab test or an X-ray, and pick up your medications – all without leaving the building.

Once you register on kp.org, you can use My Health Manager to make routine appointments at Kaiser Permanente facilities online. If you prefer, you also can call your Kaiser Permanente facility to make or change appointments. You can find the phone numbers in our facility directory.

You can get same-day, after-hours, and weekend services at many of our locations. You’ll also find valuable health information for self-care on our Web site.

We cover 24-hour emergency care from Plan providers and non-Plan providers anywhere in the world.

At many of our facilities, we have Urgent Care departments and provide same-day appointments. You can also call us to speak to an advice nurse who can help assess your symptoms, provide advice when medically appropriate, and answer questions about medical problems.

Yes, and we make it simple for you. Call the Kaiser Permanente pharmacy near you, and provide your prescription number(s) and the name and phone number of your current pharmacy. Our pharmacy team will take care of the rest! Once you’ve transferred your prescription(s), you can save time by ordering most refills online. And with our mail-order program, you can get most of your refills mailed to you at no extra charge.

Yes. Learning more about your health is a part of a well-balanced life. As a Kaiser Permanente member, you can choose from a variety of healthy living classes and other resources on topics including:

If you currently have Kaiser Permanente FEHB Self and Family enrollment, you may use our Kaiser Permanente Enrollment Change Form to complete this action. This form can be downloaded from the Tools & Resources: Brochures and Forms > page.
If you need to change the enrollment type from Self Only to Self and Family (or vice versa), you should submit an FEHB Standard Form 2809 to your agency’s human resource office. Please follow the instructions for employees and retirees on OPM’s website.

If you are adding a foster child to your current Kaiser Permanente FEHB Self and Family enrollment, you must first establish the child’s eligibility through your federal agency’s human resource office. Please refer to OPM’s Guide to Adoption and Foster Care Programs for Federal Employees for information.

Eligible emergency services are covered outside of your home service area – anywhere in the world; however, you must receive non-emergency or routine care within your home service area under your Kaiser Permanente benefit plan.
You also have access to a limited Visiting Member benefit when temporarily in another Kaiser Permanente service area or a limited Travel Benefit for continuing or follow-up when outside of your home service area by more than 100 miles. Please refer to your Kaiser Permanente plan’s FEHB brochure’s Section 5(h), Wellness and other special features, for information.
